The position of the waves, in your test examples, is still different for IE and Firefox. Safari looks similar to Firefox. I've not tested Chrome or Opera.
Because of all the browser incompatabilty headaches, I'm inclined to do some graphics processing on the smiley - so that a single .gif image results with the waves as part of the same image. The mirror code wouldn't need much tweaking to do that.
That way, any browser that can display gifs would show the waves in exactly the same position, relative to the smiley.
Do we think there will be a long-lasting requirement for the waves? It's not just a passing fad, I suppose?

oh right. I was not thinking about browser compatibility.
eta: if you're not signed in, you don't see the waves at all. Presumably guests can't access attachments. Presumably also a long-term solution would used a fixed image on the site, not an attachment.

If the enclosing relative div isn't high enough, both Firefox and Chrome clip the bottom of the smilie (when it's at the end of the post body)

The conclusion of the subcommittee on image alignment using coloured boxes is that you don't need to do that. If you use a raw img tag, not inline smilietext substitution, all browsers seem to play nicely. ( Opera and Safari still to be checked.) (I vaguely recall there is a known bug in IE to do with image vertically-centred alignment, which vB uses for substituted smilies.)
Retrieve the smilie and get its dimensions; calculate the top and left that's needed; also calculate the overall height. Then put it all in a
{div relative overall-height}{div absolute}{img waves}{/div}{div absolute top= left= }{img smilie}{/div}{/div}
